Equipment Request Form - Important Notice

Due to circumstances in relation to a third party provider, MND Victoria will not be able to deliver its usual high quality assistive technology (AT) provision for some items.

Our logistics provider, K Care, is the subject of receivership action.  At this time, access to our AT stored by them is unavailable until further notice.

This mainly affects manual wheelchairs, hoists and slings, standard and tilt in space toilet commodes, electric riser chairs, electric beds, pressure cushions and some non-retrievable items.

MND Victoria is receiving and actioning requests for AT that we normally have available.  Urgent requests should be identified on the AT request form.  Where that AT item(s) is subject to supply issues, MND Victoria will take appropriate steps to acquire and supply AT that is deemed urgent, essential and addresses safety issues for clients and carers.

We apologise for this interruption to service.  We are endeavouring to address the issues we are facing.  We hope to resume normal service shortly.

Until then, our focus, as it has always been, remains on people living with MND and their AT needs.  We will do all that we can to ensure that people have access to all AT items that they need.

We will keep you informed via amendments to this notice of our progress.

Rodney Harris
